Please use the full, legal names for all persons, including any middle names. If you have more than one middle name, include them both/all in the middle name box.
As above, this information is for submission to the Registrar and will appear on the wedding certificate. We need the name of at least one parent, whether male or female. You can choose whether or not to include the other parent. You can also chose to include one or both stepparents, if that is applicable, and if you consider that to be appropriate. If you decide to leave out a name on any of the form's required spaces, simply write n/a.
